Job summary

An exciting opportunity to join the Home Hospice Care Team as a Patient Support Worker has arisen. The postholder will provide care to people who are either likely to die within the next 2 weeks and are at home or require short term crisis support at home.

Main duties of the job

The Home Hospice Care Team (HHCT) is looking for Patient Support Workers to join our community based team.

The Home Hospice Care Team sits within the wider Oxfordshire Rapid Intervention for Palliative and End of Life Care project, as a separate team. The Home Hospice Care Team primarily provides care to people who are likely to die within the next 2 weeks and are at home. The team is also required to provide short term support to people who are in crisis.

The team operates 7 days a week from 7am-10pm and our staff work a mixture of early (7am-1.30pm) and late (3.30pm-10pm) shifts, including every other weekend. All staff have a set 4 week rota.

The team covers the whole of Oxfordshire, so it is essential that the post holder has a full UK driving licence and a reliable car available for work.

The Home Hospice Care Team has the following two bases, Unipart Business House and Horton General Hospital. You will be assigned to the base closest to where you live, for mileage purposes but will be required to cover the whole of Oxfordshire.
