Job summary
An exciting opportunity has arisen for a motivated and enthusiastic pharmacist to join the anticoagulation and thrombosis team on a 1 year fixed term period. You will work alongside healthcare professionals delivering compassionate excellence and promoting medicines safety.
The post holder will be an integral member of the Medicines Safety Pharmacy Team and the Anticoagulation and Thrombosis MDT. This post includes split duties between the local Anticoagulation Optimisation Support Service (AOSS) to primary care and the clinical anticoagulation role within Oxford University Hospitals NHS Foundation Trust.
The successful applicant will need experience within the field of anticoagulation and a proven record of multidisciplinary working. You will need the confidence and skills to succeed working in unfamiliar and challenging environments. Excellent communication and organisational skills are essential. You will work closely with haematology consultants and specialist nurses to deliver care to patients with bleeding and thrombotic disorders.
The post holder will have a focus on optimising anticoagulation for patients poorly controlled on warfarin and safe prescribing of DOACs. By mutual agreement the successful applicant would lead on agreed aspects of the inpatient role such as VTE prevention or haemophilia.
Main duties of the job
- As part of the AOSS to deliver on the pharmaceutical aspects of anticoagulation and thrombosis
- To demonstrate good clinical knowledge, accepting and managing case referrals for medicines optimisation from primary care colleagues in Oxfordshire and via the internal thrombosis MDT
- To be responsible for the review of patients on warfarin therapy whose anticoagulation control is poor
- To unify and simplify anticoagulation guidelines within primary care
- To contribute to educational teaching and provide clinical support for GPs and pharmacists including Direct Oral Anticoagulants (DOACs)
- To provide an email and telephone advice support service to deal with complex anticoagulation enquiries from healthcare professionals within primary care
- To measure the impact of interventions made by the AOSS
- To participate in anticoagulation clinics primarily for those patients on warfarin with poor control
- To be a member of the Medicines Safety Team
- To support the Pharmacy Education and Training Team to ensure the competency of all pharmacy staff who are involved with the prescribing, supply and administration of anticoagulants.
- To be an active member of the anticoagulation and thrombosis team and ensure continuous two-way exchange of information across the interface and with key stakeholders
- To work as part of the Trust's anticoagulation team (adults) for the pharmaceutical aspects of anticoagulation and pharmacological thromboprophylaxis
