Job summary

Our Trust is a nationally designated Intestinal Failure centre for both type 2 and type 3 patients. We are looking for a motivated and enthusiastic pharmacist to lead our nutrition pharmacy team based at the Churchill and John Radcliffe hospital sites. This role entails leading on the pharmaceutical aspects of adult clinical nutrition for both inpatients and patients requiring nutrition at home.Applicants will have completed a post-graduate diploma and be able to demonstrate solid foundation level practice with experience of adult parenteral nutrition. Ideally, we would like applicants to be registered as a non-medical prescriber although that is not essential. For the right candidate, there may also be the possibility of future opportunities to become involved in research.

Applicants that do not meet all essential criteria may be considered for a Band 7 to 8 progression within this post.All new appointments to the Trust are subject to a 6-month probationary period.

Main duties of the job

Clinical Practice Role:

To maintain a clinical expertise and develop particular areas of interest as appropriate and in accordance with the Trust and Pharmacy strategies. You will be providing and receiving highly complex information often in situations where there may be barriers to understanding. Requirement to communicate highly complex drug or medicine related information to prescribers, clinicians, patients: patients may have language difficulties, physical or mental disabilities and other professionals may challenge advice.

To provide a ward-based clinical pharmacy service according to Trust agreed standards including:

This role involves having access to dedicated office workstations but the majority of ward rounds will be conducted in clinical areas such as wards and intensive care units.

Actively participate as a member of daily inpatient PN ward rounds that take place on different hospital sites, including patient assessment involving for example independent clinical assessment of patients and establishing highly complex facts requiring analysis, interpretation and comparison of options.

Will require skills and knowledge for analyzing drug, patient specific information in clinical nutrition to provide advice on medicines, dosages, formulations, and production issues. May also have to make decisions in areas where information is lacking and medical or other opinion differs.

Attend and support nutrition MDT meetings, for both inpatients and outpatients.

Review invoicing related to PN services and liaise with pharmacy homecare and commissioning as required. Participate in SHIFNET and/or BPNG and/or other local/national group related to PN provision.

Provide teaching regarding PN to pharmacy, nursing, medical staff and other allied healthcare professionals (AHPs).

Be involved in the production of local and regional guidelines, when appropriate.

To provide clinical pharmacy cover to other ward areas within the team (or if required between pharmacy teams) on a reciprocal basis.

To ensure medicines are used appropriately, safely and cost-effectively in accordance with Trust policy, standard operating procedures and medicines legislation.

To monitor the use of parenteral nutrition and use of other medicines. This includes recording of significant clinical interventions and risk management including:

  • Participation in investigating clinical incidents
  • Recording significant clinical incidents/near misses
  • Ensuring compliance with medicines legislation and local policies

To use available information to influence prescribers and ensure the most cost-effective choice of therapy.

To contribute to the monitoring of safe, effective and economic use of medicines in the directorate in accordance with national and local guidelines, working with the Pharmacist Team Manager and Medicines Effectiveness Pharmacist. To participate in reviews of medicines for formulary applications, Medicines Information Leaflets and as part of Patient Group Directions relevant to the Directorate.

To develop good working relationships and communication with clinical and managerial staff within area of specialist practice and act as a link regarding all aspects of the Pharmacy Service and Medicines Management.

To attend relevant clinical unit meetings as appropriate.

To contribute to the development, implementation and monitoring of protocols for medicines related to adult clinical nutrition, particularly for high cost, high use and high risk medicines, in response to clinical and business needs of the Trust and Directorate, ensuring these protocols are integrated across the Trust as appropriate.

To provide financial information that is accurate and identifies trends.

To liaise with external organisations and agencies to ensure safe, timely and appropriate supply to patients, e.g. homecare providers of parenteral nutrition, other hospitals, and community pharmacies, etc, where appropriate.

To support the appropriate management of clinical trials and unlicensed medicines.

To support the implementation of medicines related policies such as reuse of patients own medicines, green bags etc across the Directorate.

To be aware of developments within own area of practice, the Directorate and Pharmacy/Medicines Management in general, to develop own area of specialist practice, to share good practice and act as a resource.

To tutor rotational clinical pharmacists undertaking the Diploma in Clinical Pharmacy following formal accreditation as a tutor by the relevant university, and to tutor other pharmacists and pre-registration students in the clinical area.
