Job summary
This 10 PA post comprises 8.5 direct clinical care (DCC) PAs and 1.5 SPAs. The post holders' principal duties will be at the Oxford University Hospitals NHS Trust; outpatient activity will be predominantly at the Churchill Hospital, with some clinics at the Horton Hospital and the inpatient work based at the John Radcliffe Hospital; it is a condition of the appointment that the post holder will be willing to work in any of the Trust's locations both on an inpatient & outpatient basis.
The clinical component of this role will provide senior expertise in managing patients with ILD & respiratory medicine. The Oxford Interstitial Lung Disease Service is a regional specialist ILD centre & an anti-fibrotic prescribing centre, delivering comprehensive care to the Oxfordshire and Thames Valley region. The post holder will play a key role in the care and diagnosis of patients with Interstitial Lung Disease, with the support of the ILD MDT. The post holder will also support the clinical administrative responsibilities associated with the delivery of a regional ILD service. PAs are allocated to ILD, IPF, sarcoid, combined rheumatology and general respiratory clinics, ILD MDTs, referrals, bronchoscopy & respiratory ward duty (7 weeks per annum; 24 beds). The clinics are generally set up as 3 new/9 follow ups, the service received 900 new referrals in 2022 and has 3000 patients under follow up. The ILD service contributes to clinical trials, run via the Oxford Respiratory Trials Unit.
Main duties of the job
The post holder will contribute to the provision of the respiratory medicine clinical service within the OUH NHS Foundation Trust on all sites. Clinical duties include inpatient, respiratory referrals, bronchoscopy lists and outpatient duties. The post holder will work closely with consultant colleagues to provide respiratory opinions for the patients admitted under the care of other acute general physicians on request, and to improve patients' access to respiratory diagnostics and ambulatory care. Job plans are annualised, with time in lieu in non-ward weeks to account for out of hours work when on ward duty. When on ward duty (7 weeks per annum), 3-4 nights are on call; consultants lead daily weekend ward rounds. The post holder will be provided with secretarial and IT support.

The ILD service is supported by three radiologists and two pathologists with an interest in ILD, academic and clinical rheumatologists, research nurses, and when required three on-site thoracic surgeons. Community respiratory nurses provide ILD community palliative and psychosocial support to end-stage ILD cases. Allied support to the ILD service includes physiotherapy outpatient support for pacing advice and breathlessness control, the home oxygen assessment service provide ambulatory assessments and community oxygen follow-up, direct access to pulmonary rehabilitation (Oxford city and >5 county sites), and access to a senior respiratory pharmacist with an ILD interest. Further support is provided by a comprehensive lung function laboratory, and BAL differential cell count service. The OUH Trust biologics unit facility delivers cytotoxic, immunosuppressant and biologics therapies.
