Job summary
A new opportunity has arisen for a medicines management technician to develop their skills as part of the ambulatory pharmacy team. This role will be working along side the pharmacy and clinical teams to deliver medicines optimisation to patients under the ambulatory teams including Ambulatory assessment unit (AAU) Hospital at Home, Complex Outpatient Antimicrobial Therapy (COPAT) and CovidCare@Home (CC@H)
This post would suit somebody with either a hospital pharmacy background, or with community pharmacy experience wishing to make a change into the hospital environment, for which appropriate training will be provided. You must have successfully achieved the relevant units of pharmacy services skills NVQ level 2 (QCF) or an equivalent GPhC accredited training programme.
As part of this role you will be expected to undertake home visits to patients under the care of the hospital at home team. It is therefore essential that you have access to transport
Main duties of the job
- To improve the quality of care to patients in the ambulatory clinical area (including AAU, hospital at home, COPAT and CC@H) by means of an efficient medicines management system. This will include the use of patient's own drugs, providing original pack dispensing, near-patient discharge systems and counselling patients about their medicines.
- To provide a pharmacy service that ensures the safe and cost effective use of medications.
- To work as part of the dispensary team to facilitate the efficient and effective discharge of in-patients from their clinical area out of the hospital environment.
- To be an active member of the Pharmacy department and ensure continuous two-way exchange of information to maintain high standards with the Trust.
- To under take home visits to patients to provide medicines reconciliation and counselling on medicines

- To be part of the ambulatory pharmacy team providing medicines management to the ambulatory outreach team (including hospital at home, COPAT and CC@H)
- To support medicines supply through the AAU satellite pharmacy, including stock management and ordering of medicines for use in the satellite.
- To undertake independent medicines reconciliation for patients under the hospital at home service using telemedicine or home visits where appropriate.
- To provide counselling on newly initiated medicines under the hospital at home service using telemedicine or home visits where appropriate
- To liaise with primary care including GP practices and community pharmacies to communicate medication changes and ensure the appropriate and ongoing supply of medicines for patients under the hospital at home service.
- To appropriately document medication related issues or changes identified during patient consultations in the patients electronic patient record
- To liaise with nurses and pharmacists at Oxford Health NHS Trust to ensure timely medication supplies are made to patients under the Hospital at Home South team.
- To assess medication compliance aids for appropriateness of use while patients are under the hospital at home service
- To transport medicines to patients using their own vehicle where appropriate in line with the Safe storage of medicines procedures.
- To escalate to the ambulatory pharmacists where patients require pharmacist input.
- To coordinate the repeat dispensing of prescriptions of patients requiring long term antibiotics under the COPAT service
- To be responsible for the maintenance and updating of ward stock lists in conjunction with the specialist ward pharmacist and ward staff.
- To be responsible for ensuring expiry dates are checked, expiry stock managed appropriately and stock level checks are conducted in the respiratory wards and out-patient areas.
- To assist the Operational Manager and the ambulatory pharmacists in the investigation of incident reports and changes to working practice as a result of such investigations.

- To demonstrate strong interpersonal skills and adapt appropriately in order to effectively communicate with patients and relatives, who may have visual or hearing difficulties or during difficult personal circumstances.
