Advanced Rehabilitation Assistant Early Supported Discharge Stroke

Job summary

The Early Supported Discharge (ESD) for Stroke Service delivers stroke specific therapy to individuals in their own home as an alternative to hospital based rehabilitation.

The ESD service is recruiting a full time (37.5 hours per week) (part time hours considered) Rehabilitation Assistant to join the Early Supported Discharge Team. The teams office is located at the Oxford Centre for Enablement (OCE) however the role involves supporting patients in their home environments across Oxfordshire and remote working from your own home. The post holder may initially start on a Band 3 pay point and have the opportunity to progress to a Band 4 position once agreed competencies have been reached. Support and training will be given. The ESD team is a 6 day service and the successful applicant will be required to work occasional Saturdays on a rota basis.

The post holder must have use of a car for work purposes.

Main duties of the job

The successful applicant will work closely with Occupational Therapists, Physiotherapists, Speech & Language Therapists and Nursing to support the delivery of rehabilitation and recovery of stroke patients in their own homes across Oxfordshire and participate in weekend working. The post holder will assist the ESD team with a range of duties delegated to them by therapists from each of the disciplines.

They will provide holistic interventions for patients, often without direct supervision, and will be accountable at all times to a registered practitioner. Delivery of interventions needs to be timely and responsive due to the needs of the client group, and effective team working is a key requirement of this role.

The service is closely linked with the Acute Stroke Unit at the John Radcliffe Hospital and the post holder may occasionally be asked to participate in treatment of inpatients with stroke at the John Radcliffe Hospital as the need arises.

A proactive and creative approach to achieving learning and development objectives will be an advantage. The post holder should be an enthusiastic, self-motivated and well-organised individual who is able to show initiative.
