Job summary

An exciting and challenging opportunity has arisen for a full time Band 7 Specialist Nurse to join our team. We are looking for an enthusiastic, committed Registered Nurse with recent experience of supporting people with long term conditions. You need to have good communication skills, be flexible and organised in your approach and have the ability to prioritise workloads efficiently: Evidence of working effectively as an individual and as a member of a Multidisciplinary team is essential.

Main duties of the job

The Oxfordshire Community Neurology Specialist Nurse Service (CNSNS) currently supports people living in Oxfordshire and south Northamptonshire with a confirmed diagnosis of a long term neurological condition. The service provides support closer to home, where and when it is need it through Nurse-led clinics and Home Visits. Some of the conditions the CNSNS support include, Typical and Atypical Parkinson's disease, Multiple Sclerosis, Epilepsy, Huntington's disease.

The Team also provides specialist support and education to carers and other health and social care professionals in the community. Although community based, the Specialist Nurses working within the service, work very closely with the Hospital Teams (in particular Neurology and Geratology) and the hospital-based Specialist Nurses in the Neurosciences Department. Whilst autonomous practitioners, the Specialist Nurses have a great amount of support from the clinicians in the acute trusts and lots of opportunities for collaborative working in order to develop services for supporting people with LtNc in the region.

Aims and responsibilities:

  • Patient group includes those who have a confirmed diagnosis of a LtNc and who are over the age of 18 years of age. The CNSNS covers Oxfordshire and South Northamptonshire and supports patients locally to home either in a Specialist Nurse-led clinic or in the patients own home
  • To facilitate the assessment, planning, evaluation and delivery of on-going care for a caseload of patients in order to obtain the best possible outcome in the most efficient and cost effective manner. The CNSNS works as a team to provide on -going support, where needed, to patients and service users when members of the team are on leave
  • To provide education, Support and guidance with self- management for patients, families and carers from diagnosis through to end of life. This may include supporting the local voluntary organisations
  • To provide a professional link between hospital, community services and the voluntary sectors. This may include support at meetings held by local voluntary organisations and community services, as well as supporting and attending meetings within the acute trust
  • To influence through leadership the development of knowledge, research, ideas and work practice sharing these and acting as a specialist resource for other health and social care services throughout the county
  • To independently manage a patient caseload, including: the accepting and prioritising referrals from health care professions, working autonomously in clinically assessing symptoms and response to treatment and referring to other services appropriate to patients ongoing needs (such as AHP, social services). To discharge patients only when the service is no longer required and/or other services are more suitable to provide ongoing support
  • To work autonomously managing own workload, planning own time effectively and efficiently to ensure that assessments and documentation is recorded to the required standards, within specified timescales
  • To show an understanding and responsibility to work within the guidelines set by the Trust Lone Working Policy and CNSNS risk assessments
  • As the post holder will be required to use their own car for work, it is the responsibility of the post holder to ensure their car is safe and road worthy, and vehicle service and MOTs carried out when advised. Post holders should also ensure the insurance company is aware the vehicle is to be used for the post holders work. The Team Lead will be responsible for checking that insurance and MOTs are up to date on an annual basis. Insurance certificates will be required when claiming mileage

  • To support the Team Lead in maximise staff resources in response to referrals, community clinics and education/training

  • To practice as an autonomous clinical specialist, by initiating and co-ordinating assessment, planning, delivery and evaluation of the holistic needs of patients/families with LtNc through evidence based practice
  • Acting within organisation professional and legal frameworks, to undertake to prescribe for patients in the care of the post holder
  • To facilitate self-management and support networks for patients with LtNc in the community
  • Act as a role model and resource in expert clinical evidence based practice
  • To be aware of the administrative process of patient affairs including; advanced care planning, Power of Attorney, DVLA notifications and understanding what help and support there is with benefits for those with recognised long term conditions and assist or sign post where applicable
  • To undertake assessments and provide recommendations when required for NHS Continuing Heath Care Funding for those who may meet the criteria with identifiable primary health care needs
  • Network locally, regionally, nationally and internationally and seek opportunities to develop the Specialist Nurse role
  • To promote and facilitate a learning culture, enabling other professionals to develop evidence based specialist skills such as those working in community hospitals and nursing homes
