Job summary

This is a 12-month fixed-term post as a Trust Doctor in Oxfordshire Sexual Health Service. The post starts from August 2022. The post would particularly suit doctors considering a career in genitourinary medicine, community sexual and reproductive health, obstetrics and gynaecology or General practice or those doctors with an active interest in this area or desire to broaden their experience. Doctors taking a career break have also found this to be very useful experience.

Main duties of the job

The post holder's will be based primarily at the Churchill Hospital but it is a condition of the appointment that he/she is willing and able travel to work in any of the Trust's locations (this includes all peripheral sites).

The post holder will be expected to:

  • Take part in the rota to staff routine telemedicine sexual health clinics for unselected male and female patients (with level of supervision appropriate for experience and competence).
  • Work in peripheral clinics as well as the hubs.
  • May complete training in basic contraception care up to the level of DFSRH within the department (if not already acquired) in order to deliver contraception up to at least level 2.
  • Provide holistic sexual health care including contraception, STI testing, management of sexually transmitted infections, provision of PEPSE, sexual health advice and health promotion with appropriate supervision and training.
  • Work as part of the multidisciplinary sexual health team to provide an effective and safe sexual health service. This will involve attending the department's clinical governance meetings and participating in quality improvement projects, audit and service developments.
  • Work flexibly up to 1900h (for satellite clinics, sessions may end slightly later) and one Saturday morning clinic per month (prospective cover)
  • Maintain patients' electronic records to Trust standards

In April 2014 Oxfordshires GUM and contraceptive services merged, providing a single comprehensive level 1-3 sexual health service across Oxfordshire. Consultations by a mobile student population represent a significant proportion of the clinical workload in Oxford. There are two level 3 hubs: one at the Churchill in Oxford and the other based in a Health Centre in Cope Road, Banbury. With the advent of COVID the service rapidly transformed to a new model of working and all peripheral sites were closed. A comprehensive online STI testing service, for asymptomatic patients was implemented. The first point of contact for most patients is through our telemedicine service staffed by a mix of experienced clinicians, including doctors, nurses and health advisers. We have now re-opened our peripheral sites at Witney, Didcot, Bicester, Abingdon and Wantage as well as continuing to provide telephone consultations.

PrEP clinics, HIV outpatient services, psychosexual services and some community gynaecology services are also delivered within the same department.

CLINICAL SITES

The Hubs

The Churchill is the main hub for operational and clinical administration, the coordination of services, training and clinical governance. Both the Banbury and Churchill sites offer a full clinical service for the management of sexually transmitted infections, Level 3 contraception, specialist clinics (eg vulval), community gynaecology and training (for DFSRH and Letters of competence in SDI and IUT). There are booked nurse-lead wart treatment and vaccination clinics. All infection screening results are routinely sent to patients by SMS text messaging and to achieve this we have a results management service

We now run parallel contraception and GUM telemedicine clinics for the initial assessment of most sexual health patients. FtF services are offered in Oxford, Banbury, Didcot and Witney.

SPECIALIST CLINICS

Genital Skin Disease and complex GUM and Vulval

Weekly clinic are staffed by supervised speciality registrars/SAS doctors and consultants. These clinics provide a range of care and treatments for the management of the more complex GUM cases.

Contraception

Specialist clinics compromise complex contraception (referral) clinics, difficult IUD clinic for fits and removals and referral implant clinic for removals (with ultrasound). There are dedicated training clinics for DFSRH and Letters of Competence in IUT and SDI.

Psychosexual

Oxfordshire psychosexual service provides assessment, signposting and treatment of the full range of sexual dysfunctions.

Community gynaecology (subcontracted by Gynaecology)

OSHS runs a community gynaecology clinic for one-stop out patient assessment and management of benign gynaecology including heavy or abnormal menstrual bleeding, lost threads, IUS for HRT and primary care referrals for difficult cytology sampling. Ultrasound, biopsy, Colposcopy and IUD fits and removal as well as infection screening/testing are all available within this clinic to ensure comprehensive service.

Prison

OSHS provides an in-reach genitourinary medicine clinic service to Bullingdon Prison, a male category B / C prison, and Huntercombe Prison, a category C prison for men awaiting deportation.

HIV

The Department runs one of the two services in OUH NHS Foundation Trust (the other being run by Infectious Diseases). The Departments service provides regular care for approximately 350 patients. There are three dedicated HIV clinics held weekly at the Churchill, supported by a specialist pharmacist and a small cohort of patients attend the Banbury clinic for HIV care.

In-patient facilities are provided by the relevant specialties, including Infectious Diseases and Gynaecology.
