Job summary

This is a unique opportunity to join the ambulatory care services at Oxford University Hospitals. You will be part of the pharmacy team providing care to patients seen through same day emergency care and under our ambulatory outreach team which encompasses CovidCare@Home and the Covid Medicines Delivery Unit, Hospital@Home and Complex Outpatient Antimicrobial Therapy (COPAT).

Main duties of the job

This is an additional post to the team and although experience in ambulatory care is desirable it is not essential. The successful applicant will have excellent interpersonal skills, be able to work in a wide multidisciplinary team, demonstrate sound clinical knowledge in acute medical conditions, and have experience in managing medication related issues. The appointed pharmacist will be supported in developing from experienced consultants, advanced nurse practitioners and pharmacists. This post will provide many opportunities for personal and professional development, both clinically and managerially.

The successful candidate will share Oxford University Hospital's vision and should preferably have successfully completed the Postgraduate Diploma in Clinical Pharmacy Practice (or equivalent) or be working towards completing it.

Job responsibilities

1. To provide a clinical pharmacy service to the ambulatory clinical areas including ambulatory assessment unit (AAU), Rowan ambulatory unit (RAU) and the ambulatory outreach team (AOT) which encompasses CovidCare@Home, Hospital@Home and Complex OutpatientAntimicrobial Therapy (COPAT).2. To support the development, implementation and management of home administered antibody therapies for treatment of Covid-19.3. To be a medicines expert within ambulatory services, providing support on medicine safe storage, patients medication and treatment choices and monitoring, and medicine related developments within the team. To support the development of the service. To deputise for the Lead Pharmacist as needed including line management roles.4. To work closely with the pharmacy team lead for ambulatory care in the implementation, review and audit of the introduction of ward based and at home pharmacy services and new technology.5. To be innovative in supporting all aspects of medicine optimisation adapted to ambulatory patients including managing patients medicines at home and liaising with primary care colleagues.6. To provide specialist advice and counselling to patients both on the ward and through telemed consultations.7. To work closely with a ward based technician, to support the AAU satellite in providing ward based pharmacy services8. To be a pharmacy representative within the ambulatory care area and to represent the area within Pharmacy. To liaise with the pharmacy service including the dispensaries in promoting understanding of AOT services and supporting any related changes and developments.

Essential

  • Qualified to masters degree level (MPharm or equivalent)
  • Member of the GPhC
  • Post graduate Diploma in Clinical Pharmacy or equivalent

Desirable

  • MSc in Clinical Pharmacy
  • Non-Medical Prescriber
  • Member of the RPS

Experience & Knowledge

Essential

  • Working as a hospital pharmacist
  • Working as part of a multi-disciplinary team.
  • Undertaking audit & reporting results
  • Mentoring or tutoring of pre-registration pharmacists
  • Experience of working with both adult and paediatric patients

Desirable

  • Clinical experience within ambulatory services
