Our Newborn Care Unit is a centre of excellence in neonatal care. We provide level 3 care for extremely preterm infants, critically unwell infants & infants requiring a number of specialities, including surgery, neurology, cardiology and ophthalmology. The Newborn Care Unit currently has 16 intensive care cots, 18 high dependency cots and 19 low dependency cots. We are the base for the neonatal transport team - SONeT - covering the Thames Valley area.
We have an enthusiastic education team providing support to both nursing staff and pre/post registration students. We run regular in-house study days and team days.
You'll be part of a very committed and competent outreach team, who strives to deliver excellent care in the community.
We accept applications from children's nurses, adult nurses and midwives. Critical care experience is desirable, but not essential.
Main duties of the job
- To provide autonomous nursing care in the community, ensuring a high standard of individualised evidence based and family centred care.
- To facilitate a seamless transition from hospital to home for babies and their and families in accordance with Newborn Care Unit/ Neonatal Outreach guidelines, whilst adhering to the OUH Trust Lone working policy.
- To provide education and support to families in the community.
- To work as a member of the multi-disciplinary team, communicating effectively with all levels of nursing, medical and other health care professionals, both hospital and community based.
- To partake in the supervision and preceptorship/mentorship of pre-registered students on spoke placements.
- To adhere to NMC Code 2018

- Outreach
- To work alongside the Discharge/Neonatal Outreach team, the NBCU discharge planning coordinators and multi-disciplinary team to identify and anticipate the discharge of babies into the community. To facilitate the seamless transition from hospital to home.
- To work as a member of the Discharge/Neonatal Outreach Team and the wider multi-disciplinary team providing evidence-based nursing care to babies and their families.
- Can communicate effectively in demanding and sometimes challenging situations and make decisions in the best interests of the babies and families in your care.
- Has an awareness of take-home medications and is able advise parents on requesting repeat prescriptions.
- To be able to correctly calculate medication doses to ensure correct doses are prescribed
- To ensure that parents are confident and competent to administer medications at home according to NBCU and NMC guidelines.
- Ensures a high standard of safe and effective care for babies in the community with complex needs in conjunction with the Community Paediatric Nurses.
- Be able to assess the educational and information needs of families. To devise plans to ensure these needs are met when caring for them in the community.
- Coordinates and organises within the outreach team their own workload and manages this, understanding that communication is key re upcoming discharges. Is aware that all discharges should be discussed with all team members and allocated appropriately.
- Provides regular home visits to babies and their families to provide care, including weight, OFC measurements, feeding advice and jaundice measurements. Ensure results are accurately recorded. Seeking advice from the wider MDT if required, including the senior outreach team on duty.
- Support the Discharge/Neonatal Outreach Team to ensure the smooth and safe running of the team, recognising and valuing the contribution of all team members.
- Attend MDT meetings whereby there will be an opportunity to identify and participate in the discussion for any babies being discharged with an MDT team approach.
- Can communicate and educate parents being discharged home with Oxygen, including overnight traces, downloading, and interpreting results liaising with the MDT. As well as prescribing/ ordering and installation of oxygen as per unit guidelines. Is able to manage these babies and their oxygen effectively using an individualised approach.
- Can identify potential families for the short-term tube feeding programme and effectively care for these families in the community, whilst encouraging and promoting breast feeding as part of the NBCU UNICEF Baby Friendly accreditation.
- Is aware of all aspects of Child Protection, potential complex family associated issues and acts appropriately. Attends mandatory Child protection training provided by the trust every three years.
- Encourage practice that develops partnerships with parents in the community to ensure care is family centred and individualised.
- Act as advocate, always upholding the best interests of the baby and their family in the community
- Recognises the stress factors associated with taking a baby home and able to provide advice and positive reassurance to parents.
- In the absence of Lead Co-ordinator arrange and attend safe-guarding conferences and MDT meetings for complex discharge patients which could include communication with out of area Outreach Teams, arranging care packages and liaising with palliative care specialists
