Job summary

Acute Rehab Support Worker (ARSW) - Band 4

The post-holder will be working within Ward 5B at the OUH NHS Foundation Trust.

You will be expected to provide 1 to 1 patient observations, assisting with individual therapy goals, being involved in rehabilitation through holistic and patient centred care. You must be able to demonstrate empathy, compassion and the ability to cope in challenging situations. You will work closely with the ward multi-disciplinary team (MDT) and must have a desire to make a difference to vulnerable patients who require additional observations and intervention whilst in our care.

Main duties of the job

An exciting opportunity has arisen to join Ward 5B . The post-holder will be working on the ward.

The post holder will be responsible for the delivery of 1.1 holistic care and patient observations including distraction therapy, reminiscence therapy, personal care, support with nutrition / hydration, mouth care and prevention of deconditioning. This will be with the support and the guidance of the Ward MDT.

  • To work as a member of AGM inter-disciplinary team in supporting the rehabilitation ofpatients who are admitted to the ward.
  • To perform a range of duties as detailed below under the support and guidance ofnursing and therapy staff as part of the patients rehabilitation programme
  • Provide supervised programmes of therapeutic activity for patients, within the frameworklaid down by medical staff, therapists and nursing staff to deliver a rehabilitativeapproach during all patient interactions
  • Increase the patients activity levels over 7 days to maximise carry over of learnt skillsfrom one week to the next and therefore improve outcomes.
  • Increase the patients ability to self-manage their condition and take responsibility forcarrying out exercises or practising set tasks which will be an essential factor in achievingpersonal optimum independence and a successful discharge.
  • Prevent the development of secondary complications associated with reduced mobilitysuch as low mood, joint stiffness, reduced muscle length and development of pressureulcers.
  • Under the supervision of therapists or nursing staff, implement and evaluate basictherapy interventions for patients in activities relating to their treatment programme
  • To record patient information accurately including the use of Electronic Patient Records.
  • To regularly and accurately input data onto IT system
  • To support care support workers in delivering activities planned for the patient.
