This job is now closed
Job summary
The team seeks to minimise the adverse physical and cognitive impact of hospitalisation on patients through the application of an early and active recovery model. As part of a multi-professional team, you will identify and undertake a comprehensive assessment of need at the point of hospital admission and deliver rehabilitation interventions to maximise the recovery of patients during their acute hospital stay. The primary aim of the service is to ensure patients can live well and as independently as possible in their own homes following acute hospitalisation. This will be achieved through the following interventions:
- Delivery of early intervention and support of an active recovery programme to facilitate rehabilitation and safe discharge planning.
- Implementation of programmes that minimise sedentary behaviour during the hospital stay
- Supporting prevention (where appropriate) and early resolution/management of delirium and other cognitive spectrum disorders through physical and cognitively stimulating activities
- Collaborating with local health and social care providers to minimise institutionalisation through a 'home first' approach to hospital discharge
- Engaging and supporting the healthy and active ageing theme of the department's research and development agenda through the Oxford Allied Health Professions Research and Innovation Unit (AHPRU).
