Job summary

Children's Continuing Care Team - Family Support Worker Band 4.

The Children's Continuing Care Team provides specialist care to children and young people with complex health needs in the community setting 24/7 across Derby and Derbyshire. Therefore the ability to drive/travel independently is essential.

We are looking for motivated, enthusiastic Band 4 Family Support Workers to join our team. The Band 4 position is a senior role and requires experience working with those receiving long term ventilation and complex health needs. These complex health needs vary and can range from epilepsy, enteral feeding, tracheostomy care to full home ventilation including some children with life limiting conditions.

The shift pattern you will be working will be dependent on the children and young people you are allocated to work with. See further details below.

Due to the nature of the role staff are required to sign up to the DBS update service to ensure the safety of the patients and families we care for in the community.

Main duties of the job

You will be required to work a variety of clinical shifts including nights, weekends, bank holidays and twilight shifts, we also have a number of day shifts and some long days. These shifts will be worked in the Children's Homes, Nursery/Schools, Respite Centres or occasionally at Royal Derby Hospital either as a lone worker or working alongside members of the team when 2 staff are required.

You will be required to move and handle patients up to 70kg with 2 people staff/parent and wear PPE including FFP masks when required.
