University Hospitals Derby and Burton (UHDB) Pharmacy department are looking to recruit an experienced and enthusiastic Pharmacist to join our well-respected and dynamic Antimicrobial Pharmacist Team providing high quality pharmaceutical care to our patients with a specific focus on improving antimicrobial stewardship practice across the Trust. You will be an integral part in the wider microbiology and infection prevention and control teams and support for the lead antimicrobial pharmacist. The successful candidate will be an enthusiastic and highly motivated clinical pharmacist who is able to work both individually and as part of a team. The key aspect of the role will be involvement in the antimicrobial stewardship programme in the Trust to help reduce the rise in antimicrobial resistance. You will support the team in developing a trust-wide culture that reduces hospital acquired infection and supports prudent use of antimicrobials.
The role will primarily be based at the Royal Derby Hospital site, however travel amongst all five sites will be required.
Main duties of the job
Responsibilities include:
Provision of antimicrobial prescribing and monitoring advice
Participation in microbiology MDT's and post-infection review meetings
Participation in the trust-wide antimicrobial audit programme and antimicrobial stewardship ward rounds.
Involvement in the development and review of policies and guidelines relating to antimicrobial stewardship and prescribing.
Participation in quality improvement projects relating to antimicrobial prescribing and stewardship.
Delivering educational sessions
Provide a specialist clinical pharmacy service, including evaluation of medication of individual patients for interactions, duplication etc, using medicines information and research findings to manage therapeutic problems, reduce risks and advise other healthcare professionals).
Ensure all aspects of medicines usage within antimicrobials meet agreed National or local Trust standards.
Audit of medicines use within antimicrobials.
Partake in the development of Patient Group Directions (PGD's)
Support and develop our team of rotational junior pharmacists and pre-registration trainees.
Support in the delivery of the UHDB OPAT service
You will also have a regular ward pharmacy commitment and be expected to deputise for the lead pharmacist for antimicrobials, and participate in out-of-hours and weekend rotas.
