Job summary

We are delighted to offer a position to join our Lymphoedema Team as a Patient Pathway Coordinator. This role will be based at Florence Nightingale Community Hospital in our Derby Lymphoedema outpatient clinic.

The role will involve providing an efficient secretarial and administrative service to the Lymphoedema department, including coordinating the waiting list, contacting patients to arrange appointment dates, producing letters and documents, coordinating the multi-disciplinary meetings, andother clerical duties to ensure the service runs effectively and efficiently.

Main duties of the job

  • To act as the point of contact for patients and medical staff on behalf of the consultant and his/her team ensuring that all queries are resolved with skill, sensitivity and in a courteous and professional manner. Monitoring of the phone lines and emails.
  • Responsible for the monitoring of the waiting lists.
  • Accurately informing patients/clinicians of outcomes and information to influence the effective service delivery for all patients.
  • Input patient information onto the various IT systems, including Lorenzo.
  • Contact patients to arrange appointment dates in accordance with the Trust's Booking Policy.
  • Attend, prepare and coordinate multi-disciplinary team meetings and complete actions.
  • Producing and distributing letters and associated documents to a high standard.
  • To ensure that urgent test results/correspondence is brought to the immediate attention of the Consultant or his/her team.
  • Supporting the reception team when required, greeting patients.
  • Liaise effectively with wards and departments within the hospital, as well as external parties including GPs and other Trusts.
  • Ad hoc administrative duties.
