Job summary

You will work alongside the Accident & Emergency and ward teams to provide support in ensuring the smooth running of the department and a timely patient journey. This role includes working with clinical staff and other services, working to tight deadlines and communicating with patients, families and the wider MDT.

Being a point of contact with both the ward team, patients and families is vital to help reduce the length of stay for patients.

Post will be based at Queens Burton Hospital Accident & Emergency Department.

Main duties of the job

To be responsible for the administrative coordination of the Emergency Department processes in line with the Nurse In Charge. Ensure patients, are having timely investigations, documentation, onwards referrals, admissions and discharges.

Communicates regularly with patients and their relatives to promote and maintain understanding of progress and co-operation with nursing and clinical teams to facilitate the patient journey . Is the focal point of contact for communication regarding the status of the patient within their journey as well as updating clinical teams and wider multi disciplinary teams.

Liaising with other agencies both internal and external to the trust to provide a holistic approach to the patient care. Liaises with medical, paramedical staff and with other agencies to ensure the timely delivery of prescribed patient care to facilitate a prompt timely and safe patient journey.

Ensure relevant processes are followed to enable timely admission/ discharge for patients while achieving the performance targets within the department. Early identification of potential problems that may affect the patient journey and the development of initial plans to address the issues ensuring escalation to relevant teams.
