Job summary

An exciting opportunity has arisen within the MHSOP Community Mental Health Team for a 12 month fixed term / secondment to operate as a Community Mental Health Nurse within the Older Adult's pathway within a specific Geographical area.

Main duties of the job

Working as a community mental health nurse your role will be to undertake the assessment and management of services users presenting with complex mental health care needs.You will provide a comprehensive assessment of need, offering intervention, advice and support to service users, family, carers and other professionals involved in their care.As part of the multidisciplinary team, you will ensure that care and treatment is delivered to a I high quality, ensuring that person centred care is deliver, risk are managed and appropriate treatment and other interventions (pharmacological and non-pharmacological) are offered within I timely manner.You will contribute to the effective care planning of the service user's needs by providing regular reviews, risk assessment with the goal of working towards safe discharge planning.You will practice in accordance with the Trust' Values: Respect, Everyone's Contribution Counts, Responsibility & Accountability, Patients First, Safe & High Quality Care.

  • Undertakes effective specialist initial assessment, risk assessment and rating scales to plan, implement and evaluate individualised care plans that are responsive to the needs of the service user.
  • Able to work towards recovery model including effective discharge planning and WRAP plans.
  • To facilitate in service user involvement / patient participation in area of responsibility.
  • Utilise health promotion techniques to support service users in optimising their physical and mental health wellbeing.
  • Assess manage and evaluate risk as an ongoing process using the risk assessment identified by the Trust. Escalate risks and communicate effectively with senior staff.

Qualifications / Professional Registration

Essential

  • Registered Mental Health Nurse Part 1 with Current NMC registration.
  • Successful completion of a post qualification Preceptorship programme.
  • Evidence of qualification to undertake the supervision, teaching and assessing of students (essential requirement for post, expectation this will be completed within an 18 month period unless post is bank only).
  • Evidence of Continuing Professional Development.

Desirable

  • Leadership or management qualification
  • Post-basic qualification at Diploma / Degree leve
  • Non-medical prescribing qualification or interest in working towards.

  • Relevant post qualification experience in Older Adult Mental Health Services.
