Job summary

Provide a holistic approach to patients, including assessment, diagnosis, planning and implementation of high-quality care.

To work collaboratively within the team to meet the needs of patients using evidence based care principals. Liaising with the Nursing & AHP Clinical lead, responsible for ensuring required audits are completed and all practice staff comply with local and national policy.

Main duties of the job

The post holder will provide autonomous urgent care to patients who present to the Health Centre.The post holder will:

  • Work 10 hours at Cramlington Medical Group & 24 at NPC Northumbria Way
  • Be able to work autonomously within an extended nurse practitioner role.
  • Provide a holistic approach to individuals including assessment, diagnosis, planning and implementation of high quality care up to the point of discharge or referral to another specialty
  • Support to manage the department- HR responsibilities- Contribute to the business planning process
  • Supervise and educate junior staff
  • To role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the trust. This means demonstrating a consistent leadership style which (a) engages, enables and empowers others (b) uses coaching to promoteownership of learning and quality improvement and (c) facilitates team working and collaboration within teams / departments and across organisational boundaries

About us

Northumbria Primary Care Ltd (NPC) is a pioneering partnership with local GPs and now manages several GP practices with around 120,000 registered patients in Northumberland and North Tyneside.

Whilst appointees will have a base practice within NPC,they may be asked to work at other practices across our group. We do however limit this as much as possible as we appreciate continuity is vital to effective primary care.

  • Achievement of, or working towards degree / masters' level qualification 998 or equivalent qualification
  • Teaching qualification 998,126/127 or City and Guilds 730 or equivalent qualification

Desirable

  • ALS/APLS/PLS
  • Minor Injury qualification
  • Management training course is desirable

Experience and knowledge

Essential

  • Specialist knowledge and experience gained by completion of an accredited Nurse Practitioner Training Programme or equivalent
  • Extensive post registration experience at senior grade
  • Practitioner/Nurse Practitioner qualification
  • Evidence of on-going professional development.

Desirable

  • Experience of working within an Primary Care setting.

  • It is an essential requirement of the role that the post holder has a valid driving licence and is either a car owner and able to use the car for work purposes, or has a Trust personal lease vehicle which may be used for the role.
