Locality Supervisor Palliative Care Support team

Job summary

Work across localities/PCNs to supervise, coordinate and assist with the provision of physical, personal and emotional care to patients.

Coordinating patient care resource across Localities/PCNs to provide responsive and appropriate patient care, managing the needs of patients through planned, unplanned and palliative care.

Main duties of the job

Set goals for positive outcomes as agreed with the patients.

Screens initial referral and agrees what care provision will be provided to meet needs.

Regularly review assessments of existing patients to ensure they continue to meet patient's needs.

Create and review care needs.

Allocate work to Nursing assistants.

Ensure all care needs are covered through rostering and co-ordinating nursing assistants.

Provide supervision and support to nursing assistants.

Provide sickness and holiday cover when required.

Monitor nursing assistants' training compliance.

Participate in Out of Hours Duty Arrangements, Call Outs and 'On Call' responsibilities, as necessary, to provide a Safe Service delivery.

Other duties appropriate to the nature, level and grade of the post.

To respond to enquiries both face to face and by telephone from visitors, patients and staff and other agencies.

NVQ Level 3 or equivalent in direct care/health/health and social care.

Experience with relevant patient group.

Supervisory experience.

Experience of care co-ordination and staff rostering.

A good general education demonstrating literacy and numeracy.

Ability to organise own workload and to effectively supervise work of others.

Understanding of Health policy, Health and Social Care legislation, Policies and Guidelines and their underpinning principles.

Sound understanding of the Care Quality Commission Regulatory standards.

It is an essential requirement of the role that the post holder has a valid driving license and is either a car owner and able to use the car for work purposes, or has a Trust personal lease vehicle which may be used for the role.
