Job summary

The post-holder will work as a senior community nurse, co-leading the Primary Care Workstream within a large multi-disciplinary community learning disability team in North Tyneside.

The post-holder will complete comprehensive assessments and interventions to improve the quality of lives and outcomes for patients.

The post-holder will carry a clinical case-load and have supervisory and managerial responsibility of staff.

The post -holder will contribute to service development and audit.

The post-holder will develop and deliver training.

Main duties of the job

Co-lead the Primary Care Team within CLDT. The primary care team focuses on supporting the physical health needs of adults who have a learning disability. They work in areas of health promotion to reduce the health inequalities of people who have a learning disability and improve outcomes by supporting access to primary and acute health care services.

The team support patients with epilepsy, dementia and other health conditions and offer support to the wider CLDT to help understand and embed the physical health needs of our patients into all aspects of assessment.

The Post-holder will carry a patient caseload which could also include people who have complex mental health or behaviours needs. Support for these patients is also available within the specialist forensic, mental health and positive behaviour workstreams within CLDT.

The post will help develop and deliver in-house and provider service training to improve the understanding and skills of the workforce and carers in areas of health promotion.

The post-holder will have line management and supervisory responsibility for the primary care workstream, carrying out clinical supervision and appraisals. As a senior nurse, the post-holder will provide senior support to the wider CLDT team, offering advice and guidance.

As part of the senior team, the post-holder will contribute to service development and audit for the wider CLDT.
