Job summary
We are looking to recruit an enthusiastic Support worker to improve patient's access to services to increase positive health outcomes and decrease health inequalities taking into account reasonable adjustments.
Supporting members of the multi-disciplinary team you will work with children and adults who have a learning disability and a range of health needs including mental health and behaviour that is challenging.
Main duties of the job
The successful applicant will have the opportunity to work with both children and adults, and will be involved in a range of clinical activities, including assessments, interventions and care planning in the community.

Offer early/timely intervention when there is deterioration in a patients condition by working with staff and the patient within their home offering assessment and intervention, including practical hands-on support.
To be part of the team providing a rapid response to potential placementbreakdown.
