Job summary

An exciting opportunity has arisen for an enthusiastic Registered band 6 Mental Health Nurse to join an accredited memory assessment and diagnostic service based in North Tyneside. The successful candidate will join a small team of dedicated nurses who independently assess patients with memory problems and contribute to the diagnostic process through our multi-disciplinary team meetings. You will act as key worker for patients undergoing memory assessment and support them through diagnosis. The successful candidate would also be involved in helping to run an established cognitive stimulation therapy group and participate in various other group work within the service. Venepuncture skill are essential for this post however in-house training can be provided. Knowledge of dementia and cognitive impairment is desirable however in-house training and support will be provided in post. Knowledge of therapeutic group work is desirable and the applicant will have excellent communication skills and be able to work with minimal supervision both in clinic and in the community. Community visits are occasionally required therefore a car driver with their own transport is desirable for this post. Previous post-registration experience in Elderly Mental Health Care is essential and mentorship qualification or relevant equivalent qualification is also essential for this post. Based at the North Tyneside General Hospital however you may be required to work at other sites.

Main duties of the job

Working as a Memory Clinic Community Mental Health Nurse your role will be to undertake the assessment and management of services users presenting with various types of dementia and mental health needs.

You will provide a comprehensive assessment of cognition and need as well as providing advice and support to service users, family, carers and other professionals involved in their care.

As part of the multidisciplinary team, you will ensure that care and treatment is planned and delivered to a high standard. You will oversee the initiation of cognitive enhancing treatments and when stable safely discharge back to primary care. You will also oversee the running of the therapeutic intervention Cognitive Stimulation Therapy which is a recognised treatment through the NICE.

You will provide post diagnostic support to patients with a dementia diagnosis and their families and provide effective care planning to allow patients to be safely discharged.

You will work collaboratively with the wider teams of MHSOP and with the Age UK Admiral Nurses.
