An exciting opportunity has arisen for a permanent band 5 children's nurse to join our dynamic Children's Community Nursing team who provide a wide range of nursing cares into children's homes and other community settings within Northumberland and North Tyneside.
Main duties of the job
To support the Children's Community Nurses, to enable the provision of skilled, effective and evidence based nursing care closer to home for children and young people aged 0 to 18 years with a variety of health needs in their own home and other community settings within Northumberland and North Tyneside.
To role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the trust. This means demonstrating a consistent leadership style which (a) engages, enables and empowers others (b) uses coaching to promote ownership of learning and quality improvement and (c) facilitates team working and collaboration within teams / departments and across organisational boundaries.

To support the assessment, planning, implementation and evaluation of packages of care for children and young people with acute and chronic illness, complex disability or palliative care needs.
To work in partnership with the children, their families and Carers with other professionals from Health, Social Services, Education and non statutory agencies to deliver care to meet the child and young persons assessed needs.
To provide care that is flexible and responsive to the individual and meets the demands of the service as a whole. This will include working at Special Schools and support to paediatric clinics within Child Health.
To work with young people, with complex health needs, their families / carers and others within health care to promote and contribute to a seamless transition process between children and adult services.
