Job summary
An exciting opportunity has arisen to become part of the newly established Patient Safety and Improvement team as an incident investigator. This role is being advertised on a fixed term or secondment basis to test the concept of an independant investigator.
Main duties of the job
The post holder will assist healthcare professionals across the Trust to deliver safer care through an understanding of the effects of teamwork, tasks, equipment, workspace, culture and organisation on human behaviour and identify contributory factors when incidents occur and make recommendations for systemic improvements.
The investigator will use recognised patient safety incident frameworks and tools encompassing principles of human factors and ergonomics, systems engineering, psychology and investigation best practice. A high level of sensitive engagement with patients, families, staff and others affected by incidents is expected of this role

The Patient Safety Incident Investigator will support the Patient Safety & Improvement Manager and Director of Patient Safety to provide Trust-wide oversight, assuring the quality and effectiveness of all types of patient safety incident investigations as part of the Trusts annually agreed Patient Safety Incident Response Plan (PSIRP). This is required within the Patient Safety Incident Response Framework (PSIRF) which has been implemented in response to the NHS Patient Safety Strategy.
The outcome will be the identification of systems learning from a patient safety incident that will then guide sustainable risk reduction and drive improvement, ensuring that a Just and Learning Culture and human factors principles are maintained.
The Patient Safety Incident Investigator will link in with, and provide senior leadership to, the Patient Safety and Improvement Team but will not directly manage them, as this will be the responsibility of the Patient Safety and Improvement Manager.

- Educated to Masters level or equivalent acquired through training and experience.
- Experience in coordination and completion of high level patient safety incident investigations
- Experience of delivering improvement projects and change.
- Highly developed specialist knowledge, underpinned by theory and experience
- Highly developed knowledge of patient safety and investigation acquired through a combination of in-depth experience and post graduate study
- Knowledge of change management strategies and experience of implementing change
- Knowledge of current NHS agenda and impact on services
- Knowledge of trust-wide hospitals, wards, departments, services, policies and procedures
