Job summary
We have an exciting opportunity for a team manager to be based within the CARE Point team at the North Tyneside General Hospital. This team is part of the Community Response Service.
The successful applicant will be working within a fast paced multi disciplinary team facilitating discharges from hospital across Northumberland and North Tyneside. They will work alongside the Clinical Lead and OSM to oversee the day to day management of the service.
As part of the Community Response Service, aspects of the service will be operating between 8am and 8pm, 7 days a week. There may be a requirement to work at weekend and or enhanced hours in line with the needs of the service.
Main duties of the job
- To provide professional and operational management to the multidiscipline CARE Point team ensuring the provision of a high quality therapy and discharge nursing.
- To manage a multi-disciplinary team under a locality framework to develop models of excellence in Therapy and Nursing promoting best practice within a clinical governance framework
- To be responsible for the organisation and delivery of CARE Point in the locality in line with and responding to government legislationensuring effective and efficient delivery of service
- To actively work with Clinical Lead/ Operational Service Manager/General Managers in the planning, development and evaluation of the CARE Point services, holding responsibility for defined projects
- To ensure the CARE Point service provision is prioritised and is effective within financial resource constraints
- To be responsible for the recruitment and retention needs within the defined service area
- To manage the staffing and non-staffing budget for the service
- To work with fellow team managers in the development of a high quality equitable countywide service
- To identify staff learning needs at an individual and service level and to facilitate the development of and provision of high level learning opportunities to ensure that staff meet the competencies required to deliver the service
- To take a lead on discharge planning through assessment and analysis of treatment outcomes.
