Job summary
Prostate PSA Support Worker, Band 4 (33 hours a week)
The post holder will be an integral part of the urology cancer team, who works under the supervision of the Clinical Nurse Specialist (CNS). The main purpose of the role is to support people to navigate the complex health and social care system during and/or following completion of cancer treatment. The post holder will work under guidance and supervision from clinical nurse specialist team to support patients with self-management and follow up following their treatment. You will coordinate care by delivering emotional and practical support to patients with a diagnosis of cancer.
You will be involved in monitoring patients on an active surveillance pathway who require regular blood tests, MRI scans following a treatment protocol. You will also be involved in supporting the prostate support worker in the delivery of Holistic Needs Assessment's which aim to identify and address patient's concerns.
Main duties of the job
- The duties and responsibilities listed below should be undertaken in accordance with levels of competence. In addition, under the guidance and supervision of a registered practitioner.
- The daily monitoring of active surveillance and remote monitoring patients on a database. To action when diagnostic tests and blood tests are due. To upload results onto the patient database and escalate to the registered practitioners to review results.
- To provide general information and support about cancer services, to enable people to navigate the health and social care system and make choices that are best for their cancer and their life.
- To meet the challenge of supporting patients living with the consequences of a cancer diagnosis. Encompassing patients receiving curative and palliative treatments and supportive care.
