Urology Cancer Pathway Co-ordinator, Band 5
This position is an integral part in the re-design and overall improvement of the urological cancer pathways in particular the prostate pathway in line with the national timed pathways.
The post holder will ensure that the pathway is co-ordinated efficiently and effectively, patient pathways are actively tracked to meet cancer waiting times standards in line with the national prostate timed pathway

Main duties of the job
- Contribute to the on-going development of the prostate diagnostic pathways in relation to the drive to implement the national prostate timed pathway
- To co-ordinate the patient urological cancer Prostate pathway for the Trust, and have highly functioning planning and organisational skills
- Be responsible for all administrative tasks involving in managing Prostate patient pathways
- Be responsible for day to day management, organisation and delegation of relevant tasks and roles to others as required in relation to the cancer pathways
- Be responsible for data capture and data entry for Cancer Alliance and internal Cancer Services purposes

Job responsibilities
Knowledge, Skills, Experience and Aptitude Required
- Work with DGM (Deputy General Manager) team to ensure appropriate capacity is available for <7day first seen
- Confirming that MRI report is available for clinician at clinic/MDM
- Work closely with the admissions coordinators to establish appropriate dates for diagnostics procedures.
- Liaise with the MDT coordinators ensuring patients are listed for MDT
- Liaise closely with the uro-oncology nurse specialist team to ensure the patient pathway is timely and appropriate
- To work closely with patient navigators and interdependent teams ie: radiology, pathology, cancer.
- To lead modernisation initiatives within the service line to help achieve continuous improvement in service provision.
- Attend Prostate Local MDT and Specialist MDT where appropriate
- Organise radiology or pathology review via the MDT as required
- Escalate pathology delays that could extent a patients pathway.
- Ensure Nurse Specialists and MDT co-ordinators have relevant information to enable discussion at weekly MDT meetings.
- Patients with a new diagnosis should be booked with the relevant teams for discussion of treatment and should be telephoned with an appointment date to ensure they can attend. Document any reasons for declining appointments to assist with Cancer Wait Targets. Initiate detailed tracking of these patients to ensure seamless pathway and adherence to 62 day targets.
- Ensure once patient has received a diagnosis that MDT coordinators are informed for 28 day Faster Diagnosis Standard data collection
- Following each assessment visit, liaise closely with the appropriate Team regarding each patients individual requirements, such as prior chemotherapy, chase any referral needed to progress this in order to organise their personal treatment pathway.
- Attend weekly PTL reviews with DGM
- Prioritise workload in an effective and organised manner. Be aware of clinical priorities and potential impact on the pathway, in particular with Cancer Wait Times.
- Keep track of all patients within the programme, using a customised spreadsheet, initiating action with regards to organisation of dates for treatment as necessary. Maintain the spreadsheet at all times, allowing accessibility to all within the team.
- Work with cancer services and clinical staff in re-designing the urological cancer and prostate diagnostic pathway to bring in line with national timed pathways.
- Be responsible for the accurate data entry to the South West prostate cancer dashboard
