Job summary
This is an exciting opportunity for an enthusiastic, confident, well-motivated, autonomous RGN, who works both well in a team and independently, to assist in the creation and delivery of a high-impact acute clinical service in a secondary care setting. The appointee is invited to join and develop our Acute Inpatient Heart Failure service at Gloucester Royal Hospital, assessing and coordinating the care of patients with new and established diagnoses of heart failure alongside a Band 6 Heart Failure (HF) specialist nurse, two specialist Consultant HF Cardiologists and Cardiology middle-grade doctors.
The successful applicant will be a registered Band 6 or 7 Nurse who ideally has experience of working with Heart Failure and cardiology patients but nurses with broad acute medical experience will be considered.
Experience in ECG interpretation and non-medical prescribing is desirable. The successful candidate will be a good team player and be able to effectively manage their own time and workload. The successful candidate must be computer literate, and willing to undertake training.
The appointee would be expected to attend the acute medical wards daily (Monday to Friday) to receive and identify new referrals with a remit of clarifying the diagnosis, establishing targets for optimisation of HF therapy and goals for discharge. The second 'daily role' will be to assess and treat ambulatory patients in SDEC / AEC in an 'ambulatory diuretic service.'
Main duties of the job
PATIENT CARE
- This post will support the development and implementation of a comprehensive and cohesive inpatient specialist service for patients and families of those diagnosed with heart failure (HF)
- To work closely alongside the community heart failure service and other colleagues, bridging the primary / secondary care interface.
- Provide clinical support to band 6 Heart Failure Specialist Nurses
- Ensure that patients receive care in line with national and international HF guidelines.
- To support the safe and effective running of an ambulatory diuretics service within acute care.
- To help co-ordinate the weekly 'inpatient HF MDT'
- To develop own skills and expertise so that patient's needs are met.
- To advocate for patients to ensure optimum care has been provided.
