Job summary

An exciting opportunity has arisen for an enthusiastic individual to join our Frailty/Homeward Assessment team as either a fully qualified Band 8a Advanced Clinical Practitioner or a Trainee Band 7 Advanced Clinical Practitioner who would be willing to undertake an apprenticeship pathway in Advanced Clinical Practice. The role involves demonstration of core capabilities and area specific competence relating to the four pillars of advanced clinical practice including clinical, education, research and leadership skills.

The post holder will be completing a Comprehensive Geriatric Assessment to those individuals presenting with Frailty who are attending the Emergency Department at both Gloucester and Cheltenham hospitals, Frailty Assessment Unit and Frailty Direct Attend/Same Day Emergency Care working as part of the HAT multidisciplinary team to achieve a positive outcome for the patients. The successful candidate would need to work within this fast paced environment, making autonomous decisions with the focus on admission avoidance, taking a positive risk taking approach.

Main duties of the job

To undertake autonomous complex clinical assessments and to evaluate and make clinical decisions regarding patient care and discharge pathway. The candidate would have excellent communication skills and be able to work autonomously and also liaise with the multidisciplinary team including the consultants, patient relatives and primary care partners. The post holder would be required to demonstrate and work under the four pillars of advanced clinical practice and would need to demonstrate a flexible approach as the demands of the role may change depending on the demands of the service.

Job responsibilities

- To act as an autonomous Advanced Clinical Practitioner (ACP) providing high quality, individualised clinical management for patients using advanced clinical assessment and diagnostic reasoning skills. Complete a comprehensive geriatric assessment (CGA) to determine and initiate changes to treatment as necessary within the Frailty Assessment Unit (FAU), Frailty SDEC, ED GRH and ED CGH.

- To order and interpret radiological imaging (x-rays) using findings to initiate treatment.

- Provide advice, support and education to medical, nursing and AHP staff.- Prescribe and deprescribe medication (as an independent prescriber) for therapeutic effectiveness appropriate to patient need for specific groups of patients. Working in accordance with best/evidence-based practice and national and local protocols and within the roles scope of practice and legal framework.

- Work as part of the multidisciplinary team.

- Develop and deliver educational programmes for staff locally and work with advanced practitioners nationally to develop practice.

- Act as a clinical role model/educator for developing and delivering care that is responsive to changing requirements.
