Job summary

Due to retirement and internal progression of team members we are seeking to recruit into the Integrated Discharge Team Support Worker Team. The team supports registered nursing and therapy staff in delivering high-quality, compassionate care to patients across various hospital and community settings to provide safe timely discharges from hospital.

Key Purpose of the Role
  • Provide hands-on personal care and support with activities of daily living.
  • Assist with clinical tasks e.g., observations, blood glucose recording, and venepuncture if trained.
  • Help maintain patient comfort, dignity, safety, and wellbeing at all times.
  • Provide, nursing, occupational therapy and physio therapy support as required.
  • Contribute effectively as part of a multidisciplinary team, communicating clearly with staff, patients, and carers.
  • Support with housekeeping, equipment checks, admin tasks, and maintaining a clean and safe environment.
  • Use Trust IT systems to input or retrieve patient information.

Main duties of the job

  • Work autonomy to carry out an assigned caseload of visits each day.
  • Carry out delegated routine and clinical tasks.
  • Observe, record and report changes in patient condition.
  • Assist with nutrition, hydration, mobility, and personal hygiene.
  • Transport and fit equipment assessed as discharge dependent to patients homes.
  • Escort patients between locations when needed.
  • Follow Trust policies on infection control, confidentiality, safeguarding, and health and safety.
  • Participate in training, reflective practice, and ongoing skills development.
