Job summary
As the Patient Navigator - Cirrhosis and liver cancer, you will be responsible for supporting the care of patients living with liver cirrhosis through the implementation of a database to identify and track individuals requiring treatment. You will be part of a regionally leading Hepatology service, providing excellent care to patients with liver conditions in the North East.

Main duties of the job
The post-holder will be an excellent team player who is able to work within a multidisciplinary team and able to collate and communicate data to national colleagues in NHS England. The post-holder will have proficient IT skills including Microsoft excel and hospital-based information systems. The role will include leading the implementation of a database of patients living with liver cancer. This will include using electronic patient records and data from other sources to identify patients suitable for the database.

Job responsibilities
Support the hepatology service by acting as a navigator for patients with liver cirrhosis, as well as individuals with a diagnosis of primary liver cancer (hepatocellular carcinoma HCC).
Support individuals to navigate through the pathway. This will include facilitating patients to attend surveillance imaging and helping them to access necessary services.
Maintain a liver cirrhosis database to enable the hepatology service to continue to deliver exceptional patient care and meet minimal national standards, as outlined in the relevant guidelines.
Form strong professional relationships with other clinical teams and partner agencies and work closely with them to enrol all new and existing eligible patients.
Monitor, and maintain the database, evaluating surveillance data, to ensure the service meets national standards of care and provide regular data submissions to external agencies as required.
Triage incoming calls, using a risk assessment framework and initiate appropriate response according to protocols and individual pathways, using excellent communication skills, basic clinical awareness and appropriate tools and procedures.
Consult with clinical teams to communicate and refer complex decisions for further assessment and review.
Attend MDT meetings and action planned future requests.
Ensure that appropriate surveillance, including regular imaging for primary liver cancers (Hepatocellular carcinoma) are offered in a timely manner and in line with national guidelines.
Coordinate the handover with other teams to facilitate safe and effective transition of care between services to provide seamless support for patients.
Inform patients and carers on how to make contact when they feel that their condition or needs have changed, including what to do out of hours.
Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit.
Recording, monitoring and reporting on activity and performance.

- GCSE English language and Mathematics (Grades 9-4) or equivalent qualification/experience
- NVQ level 4 (or equivalent qualification) or equivalent relevant experience
- Sound working knowledge of healthcare information systems, data collection, validation, and reporting including the ability to retrieve information from a wide range of sources and in different formats.
