Cancer Care Coordinator

Job summary

The Colorectal Cancer Care Coordinator will play a vital role in supporting the delivery of high-quality, patient-centred cancer services. Working within the multidisciplinary colorectal team, the post holder will act as a key point of contact for patients, their families, and clinical staff throughout the colorectal cancer pathway.

The role will focus on coordinating timely diagnostic investigations, treatments, and follow-up care, ensuring patients receive seamless support in line with national cancer targets and trust standards. The coordinator will be responsible for managing patient databases and cancer tracking systems, maintaining accurate and up-to-date records that support effective pathway monitoring and compliance with reporting requirements. They will proactively identify and resolve potential delays, escalating issues when necessary, and assisting with the preparation of performance reports to drive service improvement.

As part of the team, the post holder will communicate sensitively with patients, providing clear information, reassurance, and signposting to appropriate clinical and support services. They will develop strong working relationships with consultants, specialist nurses, administrative staff, and allied health professionals to ensure smooth pathway management.

Main duties of the job

The role requires excellent organisational and communication skills, proven ability to manage databases and information systems, and the capacity to prioritise a busy workload independently. Above all, the post holder will demonstrate a commitment to improving the experience of patients living with and beyond cancer, contributing to the delivery of safe, efficient, and compassionate care.

The post holder will be responsible for coordinating the colorectal cancer patient pathway, ensuring timely progression from referral through diagnosis, treatment, and follow-up in line with national cancer waiting time targets. They will act as a central point of contact for patients, offering clear information, reassurance, and support while signposting to appropriate clinical or non-clinical services.

A key element of the role will be managing and maintaining accurate patient records on cancer databases and tracking systems. This includes entering and validating data, monitoring progress, identifying potential delays, and escalating issues to the multidisciplinary team (MDT) as required. The coordinator will support the preparation of performance reports and contribute to audit and service improvement initiatives by providing accurate data and pathway information.
