Nurse Specialist - Adult Congenital Hearts

Job summary

An exciting opportunity has arisen to join the Adult Congenital Heart Disease team as a Nurse Specialist.

We are looking for an enthusiastic, highly motivated registered adult nurse with experience in cardiology. Candidates should have a keen interest in congenital heart disease.

We are seeking an individual with excellent communication and organisational skills who is able to prioritise patient care needs.

The ACHD service provides lifelong surveillance, support and education to individuals with a variety of congenital heart disease conditions. The post holder will support patient through complex congenital heart surgery including heart transplant, electrophysiology and cardiac catheter interventions, medical management of heart failure and involvement in the assessment and care of patients requiring heart transplant assessment.

The successful candidate will work within a supportive multidisciplinary team to provide expert knowledge, clinical skills, support and advice to a growing population of adults with congenital heart disease across the North East region. The post is based at the level 1 ACHD centre, Freeman Hospital, with the opportunity to attend outreach clinics across the North East.

Main duties of the job

Main duties involves the clinical management of patients with congenital heart disease both in the inpatient and outpatient settings. Including nurse led and multidisciplinary clinics, patient support and assessment during inpatient admission, coordinating and prioritising patient care and telephone triage. Participate in MDT discussion of the planning and implementing of person centered care plans.

Alongside a high standard of clinical knowledge and skills, the post demands an innovative individual with excellent organisation, time management, leadership and teamwork skills to help shape the development of a growing service. The post holder will be expected to manage their own caseload. The post holder will contribute to clinical governance within the service.

In addition to their clinical role, the post holder will be expected to contribute to the education and development of staff and students.
