Job summary

The Family Hub in Newcastle Hospitals is a new and exciting development for the Trust and will cover paediatric departments including neonates covering across Newcastle Trust inpatient sites.

The Family Hub and Complex Care Team attached to Newcastle Hospitals will support all families that are in need of extra help and support whilst their child is in hospital. These needs may not be medical in nature but may cover a range of social issues. The medical need for admission could also impact on the emotional well being of families which could as a result of long term health and social impact of a hospital admission. The ethos of the Hub will be to identify needs and respond to the family's support needs as early as possible across a range of diversity. We will offer whole family assessments using the Early Help framework to ensure that we meet the holistic needs of the family, in order to address additional wider determinants of health which may impact on our babies, children and young people, parents or carers in the hope that we mitigate against these difficulties.

Main duties of the job

To be a team member of the Complex Care Family Hub team providing clinical support into the team which includes assessing, leading and planning care using the Early Help framework, having good knowledge of support systems including voluntary agencies that may provide support to families in our care.

You will be responsible for the identification of wider support needs for babies, children, young people parents and carers.

You will have contact with children with a wide range of health needs and their families, some of which may be experiencing trauma and be distressed.

You will work directly with families to provide practical support and assistance.

You will work in partnership with other health and social professionals including voluntary agencies, parents, carers and education services to provide a holistic assessment of the needs which is child centred in preparation for discharge.

Close multi disciplinary working across many teams will be a key skill that will be required for the post.

The post holder will be a degree educated registered professional, with a sound knowledge of Early Help, understanding of the Family Help model and confident in leading team around the family meetings. You will be confident in working with parents, babies, children and young people who may have communication needs and act as an advocate bringing their voice into their journey whilst an inpatient.

This post will support safe discharge home into the families community setting and the assessment of needs to support safe discharge will start from admission, our role in supporting safe discharge will be a thorough assessment of social needs which may impact on discharge as well as liaising with the medical team for new or existing medical needs. You will have a key role in ensuring that community referrals and resources are in place in a timely manner to support families at home.

  • The Family Complex Care Coordinator will lead and manage a familys care using an Early Help Assessment and Planning Framework linking with and drawing upon services/agencies available to families to provide a whole family approach on their journey within their hospital admission and through to discharge home.
  • The postholder will organise complex discharge plans for BCYP with planning commencing from the earliest opportunity with the right interventions and include the complexities of repatriation and transfer difficulties.
