Job summary

As a Cancer Care Co-ordinator you will be responsible for supporting Nurse Specialists to support appropriate patients referred to the Nursing service within the Thoracic Medicine Unit at both Freeman and the Royal Victoria Infirmary.

You should be able to show empathy and respect to service users, have good verbal and written communication skills being able to speak appropriately within personal/sensitive situations. Observational skills will be important in this role as will time keeping/prioritising work, accuracy and ability to keeping to deadlines. Good planning and organisation skills will also be required.

Main duties of the job

An exciting opportunity has arisen within the Cardiothoracic Directorate for a permanent Cancer Care Co-ordinator. This post will support the Cancer Care Specialist Nurses within the Thoracic Medicine unit at both Freeman and RVI, duties will include:

  • Managing appointments and queries;
  • Answering and returning calls to patients, GP surgeries, district nurses, nurse specialists and secretaries;
  • Booking tests for patients undergoing investigations;
  • Complete referral proformas;
  • Ensure patients have follow up plans;
  • Prioritise workload;
  • Tracking patient pathways through cancer journey.
  • Work under supervision of nurse specialists;
  • Coach Patients and carers on how to make contact;
  • Complete holistic needs assessments with patients.

  • Triage incoming calls, using a risk assessment framework and initiate appropriate response according to protocols and individual pathways, using good communication skills, basic clinical awareness and appropriate tools and procedures, liaising as appropriate when non routine and refer complex decisions to the team for assessment and review.
  • Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit.
  • Coordinate the care for a defined group of patients assessed by a registered practitioner.
  • Organise and prioritise the designated workload in relation to identified needs.
  • Contribute to the holistic needs assessment and the development of an individual care plan.
  • Coordinate the handover with other teams to facilitate safe and effective transition of care between services in order to provide seamless support for people.
  • Deliver patient-centred, self-management support and education as necessary to non-complex patients, including how to self-assess.
  • Coach patients, carers and appropriate staff to understand what signs, symptoms or situations to be aware of that would indicate concern.
