Job summary

The Newcastle upon Tyne Hospitals has a unique and exciting opportunity to work as part of the Discharge Team. We are keen to recruit several Discharge Flow Coordinators to work flexibly on either a full or part-time basis. This role will play a key role in supporting the Trust's commitment to improve the quality of patients discharge planning and ensure patients do not spend time in hospital when they could be at home.

As a Discharge Flow Coordinator, you will be responsible for a case load of patients, visit wards daily to talk to patients about their discharge planning and ensure all plans and tasks are in place. You will be trained to have kind and caring conversations with patients whilst they are still in our care. With excellent listening skills you will accurately record information that our teams need to help them continuously improve discharge.

Main duties of the job

As Discharge Flow Coordinator you will have daily contact with both patients, families, and staff, to ensure patients are safely discharged home from hospital on the day they are fit to do so by supporting staff to proactively plan discharge.

You will be trained to use our new data platform OPTICA and ensure that up to date information regarding each patient's journey through the system is available daily and ensure that any problems which may impede the speed, safety and effective discharge of a patient are identified as early as possible and advised to the individual Complex Discharge nurse specialist and/or Clinical lead as part of an agreed escalation policy.

Job responsibilities

  • To support and work with the Clinical lead, Complex discharge nurse specialists, Wards, Multi-Disciplinary Health and Social Care professionals, and the Bed Management Team identifying, screening and tracking patients through the hospital systems.
  • Ensure that up to date information regarding each patients journey through the system is available on a daily basis.
  • Coordinate and contribute to where necessary, specific case conferences to progress discharge plans as part of and on behalf of the Discharge Liaison team and in collaboration with Multi-Disciplinary Teams, and Adults and Communities colleagues.
  • Ensure that any problems which may impede the speed, safety and effective discharge of a patient are identified as early as possible and advised to the individual Complex Discharge nurse specialist and/or Clinical lead as part of an agreed escalation policy.
  • To carry out duties as specified by Health and Social Care professionals such as completion of screening tools and the timely collection and collation of referral documentation.
  • To provide comprehensive clerical and administrative support to the Team as required.
  • To ensure that all National, Regional, Trust and Departmental policies and procedures are maintained within the remit of Delayed Discharges/Reimbursement protocols.
  • To act as a resource to others providing discharge pathway information to Ward Staff, Multi-Disciplinary Teams, Adults and Communities colleagues, patients, relatives and carers.
  • To organise and prioritise own workload ensuring the delivery of a high quality to patients through efficient use of time, communication and management skills.
  • Participate in setting and monitoring local, written quality standards and protocols to maintain and develop the service.
  • Use and participate in appropriate external and internal networks to enhance practice and to promote the department and the Trust.

Management role within inpatient and outpatient services:

  1. Assist ward staff where appropriate and take a lead in resolution of conflict regarding discharge planning between patients, relatives/carers, Multi-Disciplinary Teams, and colleagues in Adults and Communities.
  2. To assist with the development and progression of the service as directed by the Team Leader and Divisional objectives and strategies
