Job summary
An exciting opportunity has arisen for a Band 4 Occupational Therapy Associate Practitioner to join the Community Response and Rehabilitation Team Team, Community Directorate at The Newcastle upon Tyne Hospitals NHS Foundation Trust.
The successful applicant will work alongside a community based multi disciplinary team, working across a variety of community settings. The main focuses of the role will be; assessments, provision of equipment and rehabilitation in a home environment as appropriate. The post holder will demonstrate an ability to work independently when required and prioritise their own caseload.
The successful applicant will have had relevant experience either in the community or as a therapy assistant or technical instructor within an appropriate field and bring skills including, effective team working, highly developed communication, IT and moving and handling skills. Attributes will include being flexible, honest, reliable and willing to learn.

Main duties of the job
- Accept a caseload of patients under the guidance of qualified Allied Health Professionals in the team
- Be able to assess, fit and provide equipment in patient's homes
- Record outcome measures
- Undertake non-complex assessments and treatment plans including providing rehabilitation in the patient's home under the guidance of qualified Occupational Therapists / other Allied Health Professionals in the team
- Joint working alongside Occupational Therapists and the wider multidisciplinary team
- Be flexible and adaptable to respond and prioritise to the needs of the team caseload
- Work closely with Band 3 Support Workers and qualified Allied Health Professionals within a short term rehabilitation-focused care support service

- To undertake specific skilled support work to aid the Occupational Therapy team in the efficient and effective delivery of Occupational Therapy Rehabilitation to patients within speciality area within the framework of clinical governance
- To accept a patient caseload referred by an Occupational Therapist according to specific criteria. To treat patients according to set protocols, monitoring patient progress, modifying and progressing patient treatment programmes as required, with referral back to Occupational Therapists as/when appropriate
- To carry out basic assessment and treatment of non-complex patients as delegated by the Occupational Therapist
- To provide and deliver individualised treatment programmes
- To accurately record rehabilitation progress as per Trust documentation / database including outcome measures
- To undertake a clinical caseload within speciality area , without supervision for at least 50% of the time within protocols and guidelines set by the designated Occupational Therapist
