Job summary
As a Flow Co-ordinator you will be integral to optimising bed capacity and reducing transfer delays across Medicine. This is a dynamic role which will positively impact the front of house services and patient experience. It is a role suited to an individual with excellent interpersonal and organisational skills and previous experience in a healthcare setting.

Main duties of the job
- Attending daily ward rounds and liaising with clinical staff to identify suitable patients to be transferred and lead on patient transfers and discharges
- Ensure safe and timely patient flow
- Confident communicating clearly and compassionately to staff and patients
- Working efficiently independently and as part of a multi-disciplinary team
- Able to work flexibly in a fast-paced environment with an inclusive and patient-centred approach

In patient services - Bed management: o Coordination of inpatient admissions, transfer and discharge across and between medical wards
- Act a principle contact with patients, health care professionals and other organisations to ensure an efficient, effective and value for money service is in place, maintained and developed.
- Contribute to a consistently high standard of patient care as a member of a multi-disciplinary team.
- Work in collaboration with the Ward Manager and MDT staff to support the discharge planning process.
- To assist in maintaining patient flow throughout the ward.
- To act as a runner to ensure all necessary actions are undertaken and completed to facilitate efficient and effective discharge.
Management role within inpatient and outpatient services:
- Provides day to day management for ward clerks on the inpatient wards, when necessary.
- Provides local training & induction for these staff, when necessary
