Job summary
Fantastic opportunity to join a busy integrated discharge team, working across acute and hospital settings. Utilizing a discharge to assess model to prevent all avoidable admissions. Working alongside physiotherapy colleagues, nurses, and support workers, collaborative working with reablement and social services.
The service primarily works with patient on pathway 2 and 3 of discharge, discharge hubs are daily and fully integrated. Staff work across both acute sites and front of house in the Emergency Department
Community follow up visits and short term care packages are coordinated by the team, the team are represented at discharge planning and modelling events, quality improvements and research are supported.
This is a dedicated team, using a patient centred approach to ensure the patient flow through the hospital is as efficient, effective as is possible.
This post is based, at Arthurs Hill clinic working across acute and community sites. Service is 7 days per week, 8-6pm, weekend and bank holiday working is required.

Main duties of the job
Maintain a high standard of clinical work, developing discharge packages, to ensure patients can achieve maximum functional rehabilitation, through the setting and reviewing of rehabilitation objectives.
Utilise high level clinical skill.
Work as member of the MDT.
Deputize for team lead as required.
Provide mentorship to students.
Provide supervision.
Attend meetings as required.
Responsible for the safe assessment of patients and provision of care packages and equipment, liaising with families.
Robust communication, utilizing Electronic Patient records, including E record and S1.
Work with the Team lead to ensure quality improvement and compliance with clinical governance.
