We seek a dynamic individual with specialist skills in adult critical care and an associated specialism to join our team. The peri-operative and critical care directorate at the Royal Victoria Infirmary (RVI) comprises consultants, specialty doctors in training within anaesthesia and ICM, trust grades and ACCPs. We deliver a hugely diverse range of clinical activity across adult and paediatric theatres, peri-operative medicine, critical care, acute and chronic pain management and home ventilation services.

Main duties of the job
The post entails a significant commitment to critical care medicine (approximately 60% of job plan including a full share of the oncall) within RVI critical care. With the remainder of the job plan being made up by the successful candidates second specialty. Generally as part of the perioperative directorate this would be anaesthesia, however other specialties such as critical care transport, respiratory medicine, emergency medicine, and renal medicine would also be considered. For candidates with non-anaesthesia specialties please get in touch early to discuss with ourselves but also the individual second specialty provider.
Critical Care consultants at the RVI additionally work outside of the critical care units delivering resuscitation and acute care services to the wider hospital and also protecting the daily ward rounds from interruptions. At present there are 6 sessions per week for this activity (full day Wednesday, half day the remainder of the week). At times and out of hours, this role is performed by a senior trainee and is referred to with the acronym "Orange" (Out of unit and Resuscitation).

Clinical:
- Provision of critical care services as outlined above. The appointee will be expected to make a substantial contribution to the development of critical care services both on the RVI site and to the wider trust. It is expected that the applicant will be able to demonstrate training compatible with a major time commitment to critical care and as such the trust seeks candidates with the a CCT in Intensive Care Medicine or its equivalent.
- Fostering of relations with other specialties. There are strong working relationships in place between critical care and the base specialties we partner. These have delivered not just high quality clinical care but joint initiatives in service development, research and training.
- For the successful candidate with a second specialty of anaesthesia, provision of anaesthesia services as agreed between the successful candidate and the directorate. These may be based within either the Leazes or Victoria Wing theatre suites or cross site. The Victoria wing theatre services include neurosurgery, neuroradiology, orthopaedics and maxillofacial surgery. The Leazes theatre services include upper and lower GI surgery, general, breast and endocrine surgery, ophthalmology, gynaecology, plastic and burn surgery. Cross site theatres may include the day treatment centre at the Freeman Hospital.
- Standalone ICM CCT candidates or those in dual CCT with non-anaesthesia specialties should discuss their needs with the clinical director prior to application.
- On call will be on a 1 in 9 prospective cover basis to either the Ward 18 or Ward 38 Critical Care Unit. Note that each weekend day receives 2 PAs and thereafter is covered on call. There is split weekend cover with a colleague
- The department has a culture of ensuring patient safety within a multi-disciplinary team environment which the applicant is expected to adhere to.
Managerial:
- To participate in all aspects of Clinical Governance, making a significant contribution to multidisciplinary audit and risk management activities in Peri-operative and Critical Care.
- These activities will all be carried out in a multidisciplinary manner. Clinical Governance, Clinical Effectiveness and Audit meetings occur on six mornings and six afternoons every year. All members of the Directorate are expected to attend and contribute. These meetings are utilised to deliver Trust mandatory training.
- Additionally there are weekly departmental meetings within critical care including Clinical Governance, M+M, MDT and consultant meetings.
Administrative:
- To participate in all aspects of Clinical Governance, making a significant contribution to multidisciplinary quality improvement and risk management activities in Peri-operative and Critical Care. The aims of QI will include:Ensuring compliance with national guidelines relating to Critical Care and Anaesthesia.Helping to create an environment of continuously seeking to improve the quality of care.
- The four departments within the Directorate, Adult Anaesthesia, Adult Critical Care, Paediatric Anaesthesia and Pain Management function managerially as a single department. You will be expected to undertake administrative duties in relation to the care of your patients and in relation to the running of the Department of Peri-operative and Critical Care in co-operation with the Heads of Department, Specialty Manager and Clinical Director.
- To become involved with the financial management of the Directorate budget - at all times looking to ensure the delivery of high quality care to agreed standards, but at the same time reviewing working practices to provide the most efficient use of resources.
- There are close links with the other two Directorates at Freeman Hospital Peri-operative Services and Anaesthesia services within the Cardiothoracic Directorate. There is a collaborative approach to manpower planning, Clinical Governance and risk management, therapeutics, equipment and education.
Research:
- The Trust welcomes and encourages research as a high profile activity that compliments the service provided. The emphasis is on studies that fall within the National Institute of Health Research Portfolio.
- The Peri-operative and Critical Care Directorate have gained the accolade of becoming an Academic Clinical Directorate within NuTH with the associated access to research funding. There is ongoing CLRN activity within the Directorate and several active awards for Greenshoot and Research Capability funding within the consultant body.
- Newcastle Upon Tyne Hospitals Critical Care has a major commitment to research which is currently undergoing significant expansion with resultant opportunity.
- The RVI critical care areas have a strong track record of recruitment to NIHR Portfolio studies.
Teaching:
- The Directorate places considerable emphasis on under-graduate and post-graduate education. You will be expected to make an active contribution to under-graduate and post-graduate teaching of Anaesthesia and critical care medicine.
