Job summary

This is an exciting development opportunity for a fixed term 2-year contact to become a Nurse Practitioner District Nurse on the evening and night service.

We are seeking an experienced, innovative and enthusiastic band 6 nurse with the specialist qualification in district nursing that would like the opportunity to develop their clinical and operational skills.

The post will be temporary initially but will be extended if the successful candidate has obtained the advanced clinical skills and non medical prescribing (V300). The candidate must also be able to demonstrate strong leadership and operational management for the team/service.

The successful candidates main job role will consist of;

The responsibility and accountability for the unplanned nursing care citywide, Also Leadership, management and operational management of the district nursing team.

To engage in the assessment of individuals, and family health care needs.

To Assess and diagnose changes in the patient's physical condition prescribing medications and working within the 2-hour response, prioritising the importance and clinical need to avoid unnecessary hospital admissions.

To act as a clinical supervisor within the service to support staff.

To provide a high level of expert evidence-based practice and clinical expertise to housebound people in the community.

To Assess and diagnose changes in the patients physical condition, prescribing medications working with the Trusts policies and procedures.

To deliver a quality of service matching clinical need to available resources within the focus of specialist nursing practice:

  • To ensure that the patient is the central focus of a streamlined patient journey ensuring optimal quality of care.
  • Manage own caseload to include assessment, investigation, diagnosis and treatment within defined parameters and training.
  • Provide immediate care management of patient including communication of plan to patients and significant others.
  • Review and refer to appropriate person, e.g., AHP, GP, CRRT, Specialist Palliative Care Team, outreach team.
  • Communicate within multidisciplinary team, ward/departmental staff and/or out of hours team.
  • Provide specialist education and development of staff members and assist with the development of clinical services and enhance practice across the Trust.
  • To provide clinical leadership across the service.

Work with housebound patients in their own homes and Care Homes. Working across the whole of the city and covering patients with a Newcastle GP.

The post holder will act as clinical supervisor to peer and provide specialist clinical advice to community staff.

  • Current NMC registration

  • Master's degree
