Job summary
An experienced Band 3Community Cardiac Rehabilitation Workerto work autonomously visiting cardiac patients in their own homes. The job is to support, motivate and deliver an individualised exercise programme, that has been assessed and prescribed by our Band 7 Physiotherapist. The patients are seen by our experienced Community Cardiology Nursing Team prior to them being referred to the home exercise programme. The patients have either Heart Failure, or had a recent acute cardiac event or heart surgery. We are looking for a very enthusiastic, confident nurse to join our dynamic innovative rehabilitation Team.

Main duties of the job
To assist the Registered Nurses and Physiotherapists and work autonomously in providing high quality patient care within the Community Setting. Checking baseline observations, signs and symptoms prior to delivering the prescribed, individualised exercise plan and set goals. The job includes covering cardiac rehabilitation phase 3 group work at four Centres across the City alongside a Cardiology Specialist Nurse as required. Car Driver is essential. Ongoing support, clinical supervision and the relevant training will be provided.
