Job summary

An exciting opportunity has risen to work within theCommunity Response and Rehabilitation Team as a Specialist Nurse,we are a multidisciplinary team working in the community. It is a varied role which involves assessing patients in their own home, we also provide a two hour response for urgent visits required within the community and case management of complex patients. Our aim is to facilitate efficient discharges and reduce avoidable hospital admissions.

Weprovide support over seven days 8.30am to 8.30pm therefore you would be expected to support out of hours working to enable hospital discharges to continue over 7 days and to continue to avoid hospital admission and enable patients to remain at home.

You will require experience of assessment, planning and delivering high quality patient care and demonstrate a range of NHS experience in a variety of specialties, including older people in a community setting. You will require good communication and interpersonal skills, and take an active role in the Supervision and training of undergraduate students and support staff.

Main duties of the job

Comprehensive Assessment in the community.

Community discharge follow up visits.

Collaborative working with stakeholder organisations within the hospital and community.

Close MDT and interdisciplinary Team.

Teaching and assessing of Junior staff, other disciplines, and students.

We work over 7 days a week including bank holidays. We are looking for dynamic, enthusiastic staff who will thrive working in a varied, busy environment.

Car driver is essential.

You need to be able to work autonomously with a patient centered approach and have excellent clinical decision making and time management skills.

The team sits within the community directorate and works closely with social care teams.
