Job summary

An exciting opportunity has arisen within the Newcastle Community CHEST team, for an enthusiastic, dynamic, and motivated registered nurse/physiotherapist. You will be part of a team of nurses and physiotherapists who currently provide COPD early supported discharge, COPD exacerbation management at home and Home Oxygen assessment and review.

This post requires the applicant to have relevant post registration experience within the required specialist area of practice. You will be expected to work autonomously within both the hospital and community settings, assessing, managing and treating patients with acute exacerbations of COPD. You will be expected to visit patients within their own homes which includes working weekends and bank holidays.

Main duties of the job

The Newcastle Community CHEST Service plays a vital role in supporting patients in Newcastle who have a diagnosis of COPD. Within this, we are expanding our service to include a virtual ward service with the aim of reducing hospital length of stay for patients admitted with an exacerbation of COPD. Patients will be reviewed and assessed in hospital for suitability of the virtual ward by the team. If found to be suitable the discharge and the organisation of any additional equipment will be facilitated by the team. We will then be offering daily clinical review at home and telephone support with advice on condition management, inhaler technique, referral to other services such as Pulmonary Rehabilitation, and ensure the patient has appropriate follow up in place from the wider respiratory team.

The service will be provided over 7 days and include Bank Holidays. There will be an on call commitment to this post up to 8pm. Lone working is a significant part of the role and the candidate should be familiar with the Trust lone working policy. Lone working safety devices will be available to staff. The candidate will need to hold a valid drivers licence and have a vehicle suitable to be able to perform home visits.

Applicants should be organised, flexible with effective communication skills and able to work as part of an established, experienced team of nurses and physiotherapists.

The successful candidate would be expected to have a good broad knowledge on a variety of cardio/respiratory conditions in order to assist in the 6-week or annual review of the oxygen user, and participate in the screening and prescription of ambulatory oxygen for patients who do not meet the criteria for Long Term Oxygen Therapy.

The successful candidate will be confident in carrying out observations in the patients home setting, have excellent communication skills both with patients and their relatives but also with the wider MDT and have good knowledge of COPD and its management. The successful candidate will ideally have experience in managing the acutely unwell patient and knowledge of the escalation process should a patient become too unwell to be managed safely in the community. Clinical skills such as history taking, auscultation and medication management is useful.

In addition, we are the Home Oxygen Service Assessment and Review Team for Newcastle. We look after all adult oxygen users in Newcastle who are registered with a Newcastle GP, regardless of the indication for the prescription of oxygen.
