Job summary

We are excited to be offering a new opportunity on a temporary basis suited to an adaptable and forward thinking, experienced nurse to join our service and become an integral part of the cardiac congenital MDT.

This senior post will act as our Nurse Specialist Coordinator for patients with congenital heart disease.

Within our congenital service we support a whole life approach to care caring for babies born with a heart condition through the entire life cycle to late adulthood.

This makes the role unique and exciting as the successful candidate will embrace this ethos to ensure a positive patient experience for all our children and adults ensuring their voice is heard.

The post holder will work with the congenital MDT to effectively manage the patient surgical pathway from referral to discharge from acute surgical care.

This will include providing day-to-day oversight of these patient's bespoke admission plans to wider allocation of surgical beds across the congenital cardiac surgery footprint.

The expected impact of this role will be to increase day of surgery admissions with patients arriving fully prepared ensuring all preoperative assessment has been undertaken.

Main duties of the job

  • work in close contact with congenital MDT in a spectrum of clinical management duties to optimise and improve patients flow and experience.
  • be responsible for managing the patient population from referral to surgery through to discharge from acute surgical care, providing continuity of care through their journey.
  • work closely with families of paediatric cardiac and adult congenital cardiac patients to streamline their journey from referral to surgery through to discharge.
  • be proactive in early recognition of any additional support our patients may require.
  • take ownership of a smooth preadmission process (investigations, clinical review, consent process, family support
  • have oversight of all patients on the congenital waiting list understanding from a clinical perspective if they are ready for surgery and if not, steps required to optimise this ie dental assessment
  • oversee the congenital service from a planning perspective over the month to moving patients through, and day to day perspective for smooth running
  • gain understanding of day-to-day management and the allocation of surgical beds across congenital cardiac surgery (paediatrics and adults) in collaboration with teams and colleagues
  • support management of clinical capacity to accommodate elective and emergency admissions
  • facilitate seamless effective appropriate clinic appointments with purpose of obtaining consent, investigations, and admission for surgery
