Job summary

This is a dynamic time within the addictions and liver cirrhosis team. We are a rapidly expanding team and by joining us you will help to provide patient focused support to an often-marginalized group. This role is to enhance care provision using a bespoke approach to an individual who may be struggling to engage with their community, support services or health care needs.

They may also be involved with the criminal justice system and being an advocate for individuals often socially isolated is key. Early intervention and referral to our community partners within addiction will be a further focal part of this role.

This role predominantly sits within the accident and emergency department but also involves in-reach to patients in ward areas and contact with patients who have presented outside of working hours.

This job is part of an evaluation project of the Recovery Navigator role in the locality and is an opportunity to be involved in a research project and contribute to its outcome.

If you strive to work within a gold standard team providing individualized patient care within a dynamic evolving service, then this is for you. A knowledge of this work and supporting services would be helpful but not essential and training would be provided along with regular clinical supervision.

Main duties of the job

  • This role is currently Monday - Friday working corporate standard hours of 08.30-16.30
  • Most of the working day will be within the accident and emergency department but also into ward areas.
  • You will be responsible for keeping and maintaining data for the evaluation of the role.
  • You will be involved in a research project in partnership with Newcastle University
  • You will work closely with the Alcohol Care Team and have close supervision and the opportunity to access an extensive training plan to provide ongoing continuing professional development.
  • You will liaise with community partners including addiction, criminal justice, accommodation, and employment services.

  • Form strong professional relationships with the Emergency Department team and work closely with them to identify patients who may benefit from the Navigator service.
  • The Patient Coordinator will deliver, co-ordinate and contribute to a recovery programme for service users with alcohol misuse issues, including the delivery practical, bespoke interventions both face to face and with a network of partner agencies, ensuring that patients are offered the right interventions in the right place, at the right time.
  • They will support the patient through the journey from identification and referral to conclusion of their intervention, eliminating barriers to care.
  • The Patient Coordinator will work closely with Emergency Department Team and Alcohol Care Team, and partner agencies to ensure a patient focussed service, and will keep accurate patient and activity records to facilitate reporting back to Commissioners regarding performance and service success.

  • ECDL advanced or equivalent experience: Computer literate with a good working knowledge of Microsoft packages. Experience of using spreadsheets to a high level

  • Experience of working with substance/alcohol misuse issues and a clear understanding of the need for and ability to deliver quality services.
  • An understanding of substance misuse and violence and its associated causal factors, through professional and/or personal "lived" experience
