Job summary

The Governance & Quality Assurance Coordinator plays a vital role in supporting the delivery of safe, effective and person centred care across the Mental Health (MH) Directorate. Working within the Directorate Governance Team, the post holder contributes to a wide range of governance, patient safety and quality improvement activities. This includes supporting clinical audit, risk management, external assurance and internal governance processes to ensure high standards of safety, learning and regulatory compliance.

A key focus of this role is coordinating incident investigation processes in line with Trust policies and the Patient Safety Incident Response Framework. This includes overseeing investigation administration, supporting signoff processes, ensuring robust action plans, and leading on evidence collation to demonstrate learning and improvement and supporting the Coroner's process.

The post also provides expert advice to clinical and nonclinical staff on governance processes and obligations such as Duty of Candour. Working both independently and as part of a multidisciplinary governance team, the post holder must manage competing priorities, maintain attention to detail, and support MH services in embedding a culture of continuous improvement and learning

Main duties of the job

PATIENT SAFETY INVESTIGTIONS:

Working with the Corporate Patient Safety Team (CPST), operational and clinical staff within DMH to ensure incident investigations are completed in line with Trust Policy and Patient Safety Incident Response

Framework regulations Development and maintenance of appropriate administration systems, databases and reliable input of data Support the oversight of the directorate incident investigation sign-off process

Provision of expert knowledge and advice of the incident investigation systems and processes to clinical and non-clinical directorate staff To lead on the timely collation of action plan evidence for all completed incident investigations across DMH

To lead on meeting legal requirements for Duty of Candour and Being Open Act as the link between the LPT Legal team and the directorate to ensure the Coroner's process is completed to expected standard
