Job summary

The Community Learning Disability (LD) Nursing Team provides holistic support across diverse health and social needs. The role focuses on improving health, reducing barriers to independence, promoting wellbeing and social inclusion, and supporting families and carers. As a Band 6 Community LD Nurse, you will work autonomously, lead on safe and effective care, and help ensure people with learning disabilities can live healthy, fulfilling and independent lives. Key Responsibilities: Manage your own caseload and take responsibility for care quality, safety and clinical activity. Provide clinical leadership, advice and guidance across core quality areas, including effective interventions, patient experience and a positive care environment. Contribute to team leadership and support ongoing service development to meet patient and family needs. Act as a role model by mentoring, teaching, assessing and supporting students and colleagues. Work collaboratively with senior nurses across LPT and promote the role of LD nursing to internal and external partners. Build transparent and effective networks with stakeholders and partner services. Assess, plan, implement and evaluate evidence based care for individuals with complex and changing needs. As part of this role you will be required to hold a valid UK driving licence and have access to your own vehicle, which requires business insurance. Main duties of the job

Co-ordinate, prioritises, facilitate and monitor the delivery of care for patients on a designated caseload and to other patients as deemed appropriate. Act as a professional and clinical role model to all staff, patients, carers and the public at all times Supervise the performance standards of own team members where appropriate. Maximise use of clinical and staff resources, implementing best practice in patient pathways, admission and discharge arrangements. Plan and promote the development of new services within the designated area. Maintain efficient and effective communication on issues related to patient care, caseload and organisation management. Participate in staff recruitment and selection processes and wider recruitment and retention strategies. Act in a professional manner and ensure self and colleagues adhere to the LPT Uniform Policy at all times.
