Job summary
Leicestershire Partnership NHS Trust (LPT) is working in collaboration with Adult Social Care across Leicester, Leicestershire and Rutland (LLR) to deliver an enhanced Home Firstoffer alongside core Community Therapy services. This integrated model ensures individuals receive high-quality, timely therapy in the right place and at the right time.
Significant investment has been made to strengthen our workforce, enabling us to provide responsive, personcentred care across LLR.
This is an exciting opportunity to join a service that brings together community-based nursing, therapy, and adult social care teams to support individuals in maintaining independence and achieving meaningful outcomes at home.
As part of our therapy service, you will: Support people to remain safely at home during periods of health or social care crisis, avoiding unnecessary hospital or care home admission. Facilitate timely discharge from hospital and deliver rehabilitation and reablement to promote recovery, wellbeing, and independence. Provide high-quality core therapy interventions within individuals homes as part of a coordinated multidisciplinary approach.

Main duties of the job
You will work alongside Physiotherapists and Occupational Therapists carrying out comprehensive functional assessments and interventions in order to help our patients live as independently as possible.
These interventions may include progression of exercises and mobility assessments, assessments of activities of daily living and rehabilitation.
You will also be required to carry out general administration and clerical duties.
