Job summary
We are seeking a motivated and compassionate Occupational Therapist to join our multidisciplinary team and deliver high-quality rehabilitation within our specialist High Dependency Pathway. Supporting up to 15 adults (18+, predominantly 65+), this service provides focused assessment and therapy for individuals who are medically optimised but not yet ready to return home. Working within a dual-registered care home setting, you will play a key role in promoting the Home First philosophy and enabling safe, timely discharge. You will complete holistic assessments, deliver targeted cognitive and physical rehabilitation, and provide specialist input for individuals experiencing delirium, dementia, or cognitive impairment. You will work closely with patients, families, and the wider MDT to coordinate discharge planning, ensuring person-centred, evidence-based interventions. This includes offering guidance to care home staff to support consistent 24-hour rehabilitation practices. We are looking for an OT with post-registration experience in adult mental health, strong clinical reasoning, and a compassionate, non-judgemental approach. Main duties of the job
We are seeking a committed and skilled Occupational Therapist to join our High Dependency Pathway, providing high-quality rehabilitation for adults who are medically optimised but not yet able to return home. Based within a dual-registered care home setting, the service supports up to 15 patients and focuses on safe, timely discharge aligned with the Home First approach. The post holder will deliver face-to-face assessment, intervention and discharge planning (MondayFriday, 08:0016:00). You will complete holistic assessments of physical, cognitive and functional needs and provide targeted interventions for patients experiencing delirium, dementia, challenging behaviours or functional decline following acute illness. Therapy will be delivered on both an individual and group basis. You will participate in daily handovers and weekly MDT reviews, ensuring coordinated care and progress towards discharge within the 4-week pathway. Responsibilities include identifying and ordering essential equipment, liaising with community services for onward referrals, and supporting home-based assessment where required. The role also involves supervising unregistered staff, delivering training to care-home teams and promoting consistent 24-hour reablement practice. We are seeking an OT with post-registration experience in adult mental health, excellent communication skills and a compassionate, person-centred approach.
